Solved it! I just had to flatten out the production structure. The changes to the .htaccess files were correct, or at least the work!
If anyone would like, I have started a Google Docs spreadsheet where people could put their directory structures for various hosting sites. Perhaps seeing an example will be easier than reading a myriad of posts. Anyway, here it is if any would like to contribute. I was think each hosting site could be a separate tab in the spreadsheet, or perhaps it would be best to have separate spreadsheets. See what you think.
