It's a configuration issue with the ATA or the wrong ATA sending a T38 re-invite. Please can you use the SIP debug facility on all the relevant hosts (in your attachment is only the SIP messages between your callweaver and 2012) also so we can see exactly where in the process the process_sdp: Unable to lookup host in c= line error is issued?
If you listen in on the ATA you will probably notice when that error is issued no further audio is played.
