We use iSoftphone as a primary handset on our laptops.  It's  
reasonably stable under 10.4, somewhat unstable under 10.5.

Haven't done anything special:

sip.conf

allow = ulaw

In the above example, iSoftphone prefs:

My SIP accounts -

Username/Display Name/Authorization Name all 102
Password blahblah
Local SIP port 5060

Other -
Send DTMF codes using RTP event (RFC2833)

Haven't tried BT headsets.  We usually use a Logitech USB headset  
which is both comfortable and provides good quality.
