I suspect the problem with Hannah is two fold.
1. He had work done on rear bedroom that required removal of some pipework. 
It is a possibility that the person who did the work did not correctly 
tighten up the jubilee clips.
2. The owner does not do his own maintenance and I get the impression he 
leaves it to others. I also get the impression that he does not do any 
rudimentry checks before going onto rivers. The jubilee clip 'may' have been 
a bit on the loose side and slightly higher revs and some engine vibration 
may have cause it to come off.

I did not like how his calorifier had been plumbed in anyway.
Very short rubber hose (about 2 inches) with jubilee clip onto engine take 
off. Other end onto plastic piepwork held on with jubilee clip.
