I spoke to someone who had been involved in the H&S enquiry into the 
fatality on a community boat under Spaghetti Junction some years ago. 
Apparently in that case when the rudder hit the bank the tiller swept 
the steerer into the water where she was sucked into the still-turning 
propeller. This may have been a similar case.

The H&S view was apparently that all boats should have railings round 
the stern (I think the boat concerned had a trad stern) which would have 
been counter-productive in my opinion - fortunately it was not 
implemented.

As it happens, while helping Guy move VIRGO yesterday, we encountered a 
dayboat going up and down Calcutt locks with youngsters leaping around 
on the roof and on and off the lockside when the locks were empty. I 
warned the adults that they would only fall into the propeller once, 
with no effect!
