I've managed to re-create the problem, but only by:

1) Interrupting (Ctrl+C) the script when it asks for a MAAS account
   password (before entering the password)
2) Editing /root/.maas-cert-server/progress to remove the setup_maas_admin
   line
3) Re-running the script

The error is caused by the "maas init region+rack..." line, which was slightly 
altered in this version to get around changes in the maas CLI. (A previous 
version required "--mode=" before "region+rack", but that no longer works.) I 
don't think this change is critical in this problem, although it could be that 
an underlying change to the maas CLI is.

I've been unable to reproduce the problem in any other way. I've tried 
variants, including pausing for several minutes when asked to enter a MAAS 
password (on the hypothesis that there might be a timing issue related to the 
previous call to set up the PostgresQL database).

Adrian, did you by chance interrupt the initial run of the program in any way 
remotely similar to what I've described above? If so, can you try again with a 
fresh install without such an interruption?
