Are they separate DB instances, though? Are are all clients hosted in
the same database?

Basically, all the :primary => true constraint is there for, is to
make sure that migrations don't get run on slave databases (since that
would hose replication). If your setup is such that running the
migration on each client database would be ok (and is in fact what you
want), then adding :primary => true is appropriate.
