I've finally managed to solve this one.

I got the hosting support guys to add
PermitUserEnvironment yes
to /etc/ssh/sshd_config.

Then created ~/.ssh/environment with:
GEM_PATH=/home/myuser/gems:/usr/lib/ruby/gems/1.8
GEM_HOME=/home/myuser/gems

and now capistrano can see my gems.
