I agree Vebjorn:

1) fullboth should work as the deprecated mac-toggle-max-window and  
use the full screen, with hiding the dock. In this case I dont mind if  
other apps behave differently. I love to work with carbon emacs, and  
when I do this in fullboth mode, I dont want to see any menu and dock.  
I use this mode typically when I have one frame.

2) fullheight/'fullwidth should consider the dock and adjust the width  
and height of the dock, so that it wont slip beneath the dock. I use  
this mode typically when I have more than one frame, or when I want to  
see other app windows in simultaniously.
