Hello Matt,

what I did with the ICD10 was this:   I got the data in the text file from a 
state-funded org.  The data was formatted with tabs and spaces.
I have just written a php script which read this text file and parsed each 
line extracting only the data which I thought would be relevant to the care2x 
table. Then after every parsing of one line, the script composed the sql 
insert query and inserted it in the databank. This was relatively a bit slow 
but it worked. Anyway, the script needed to do it only once.

Regarding snail mail, yes I got the cd. I sent a private email about this.  
Hope you got it. 

I am planning to upload them online for others to avail of, but I am a bit 
wary of the name of the institution on the docs. Is it ok to publish them 
with the real name of the institution? Please I need your advice on this.
