Regarding the webcam check the www.axis.com. They have network cameras that are really cool and could help you with some of this. Also the cameras alone have software in java and activex.
btw, how do you upload an image to the server that comes from a network camera (preview in browser, click and upload to a specific patients record during the registration) ???
