It seems likely that the Continuity of Care Standards will soon be adopted and promulgated by a wide array of organizations ( http://www.astm.org/COMMIT/E31_CCRJuly04.ppt ) I am wondering if anyone is aware of open source applications that could easily take advantage of these standards for the creation of CCR documents, particularly for clinics that do not have access to full blown EHR's? Of course it could be as simple as using an MS- WORD template....<:
