4,5 - You can submit your contribution to the Subversion code repository. The code base directory tree generally follows the structure of the installation. There is some document about care2x SVN on the wiki, but I we know we have to wait a little bit to have it back. In the meantime you can check the general instructions available on sf.net [1]. 6 - I suppose you mean you are going to prepare some design document about nursing process. When you are ready, post in the mail list and it will be discussed how to implement it in care2x.
