As a first step, it might be possible to make all phonenumbers linkable with a callto: link. I know at least Skype and Netmeeting can be associated with this type of link. (my guess is that most SIP clients can handle the same link type). That solution would be a very quick win for hospitals that can manage the rollout of a PC based VOIP infrastructure. I would suggest that using this type of link is implemented as an option in care2x, so it can be switched off for less advanced hospitals. Secondly all staff could have IM fields which are clickable as well. (I would suggest to use Jabber, as it is an open protocol and very easy to install a local server). Then, we could built in a web-based jabber client, so all staff can see eachothers availability, and directly chat with eachother from care2x. If you want to do web-based VOIP, in this project (http://www.asterisk2billing.org/) in the User interface, they built in a Java-Applet that can make SIP calls. Great for maintainability. You mention that the VOIP client could use multiple providers, but for a hospital, wouldn't it be far more cost-effective to run their own VOIP server?
