If you are also looking at BI, then maybe you already know or would be interested in DHIS, a south-african project that does BI for Health Information. If you read their roadmap (http://www.hisp.info/confluence/display/DHIS2/Roadmap) you will see that this october they plan to work on a care2x integration. I definitly know that people in Tanzania (are you reading along?) have planned a similar action! Secondly, DHIS runs on a Java platform, and is planning to use BIRT. That seems like a good argument in favor of this platform, even if it means that it will make the whole configuration of Apache + PHP + MySQL + care2x + Java Application Server + BIRT + DHIS a bit more complicated.
