I was hoping to get access to an earlier version of VTK sich as you provided 
but I suspect that this is not a Windows flavor of the code, right? (Since the 
more recent versions have an install package that creates an include and 
library directory directly and I can't seem to find that in the zip you 
provided. But if you can help me get that earlier (Windows) flavor installed, 
that should get me going, I'll bet.

 

I should have added to my original posting that my goal is to experiment with 
new surface modeling and registration algorithms and see if I can make some 
improvements to Caret since it looks like a fine environment to do such work 
in. So source code is kind of core to that I think.
