Interesting. It's likely that the SAML assertion expired while the user was lingering on the warn page. The more general problem of ticket expiration while lingering on the warn page is a known issue and one we probably need to document more prominently until we can craft a fix. It would be interesting to know whether assertion expiration is the root cause of your problem.
