Fix For: 0.6
The first phase of bootstrapping (INITIATE) currently happens it its own
thread. There is no real need for this. Move the necessary code from
BS.startBootstrap into SS. The remaining static pieces of BootStrapper should
be placed in the appropriate Stream* classes or renamed as BootstrapUtils.
