Check the USER_AGENT when the error happened.  For some reason (which
I haven't bothered to dig into yet), I've got a Monorail sight that
occasionally (maybe once every two months or so) generates an error
similar to yours.  Visiting the URL returns no error.  But, it always
seems to be a spider of some kind -- either Yahoo's Slurp or Google's
or something like that.  Don't know if it's just a coincidence, but
it's never been reported by a user -- and the error has never been
captured when a user is logged in.
