We are pleased to share the newly posted PCC Instructor Report Form <https://www.surveymonkey.com/s/Q7VZRDP> with you. The report form was developed based on the discussion at the PCC Training Meeting at ALA Midwinter in Philadelphia. It is posted on the PCC homepage <http://www.loc.gov/aba/pcc/> and the Catalogers Learning Workshop homepage <http://www.loc.gov/catworkshop/RDA%20training%20materials/index.html> . The report form is used by instructors who conduct outside training on behalf of the PCC. The form is not intended for use for training conducted within the institution. Instructors have the opportunity to request a Certificate of Appreciation by filling out the form. It is important that instructors fill out the form for the PCC training they provide. The PCC Secretariat uses the information gathered on the form to capture PCC training statistics. Please fill out the form for the outside training you have done during the current fiscal year (e.g., October 1, 2013 - September 30, 2014).
