[OSL | CCIE_Voice] SRST config - BR2

I am configuring up SRST in the lab, and I get the Phone to register 
correctly and the phones show 'CM Fallback Operatiing'
However, I get no line buttons on the phone?  CCM 4.1 - IOS 12.4T 2691.
