If I go to SETTINGS >> STATUS >> STATUS MESSAGES I see where it's trying to 
load SEP<mac>.cnf.xml

However, under SETTINGS >> STATUS >> FIRMWARE VERSIONS it does tell me that the 
SIP load is SIP41.8-5-2S.

So I believe its has downloaded the correct firmware version from CUCM.

A few other things I have verified:

·         It is receiving an IP from DHCP

·         The CUCM device defaults specifies SIP41.8-5-2S for all standard 7961 
phones

·         The phone is built in CUCM as a SIP phone

The phone also says "Unprovisioned."

Any other ideas?  Am I the only one running into this?

This is the case in which I would look further into the cucm traces, (assuming 
dhcp configuration, cable, and cucm configuration is good)

However, before doing that, I would make sure that after a factory reset the 
phone is getting the correct sip phone load, by your previous traces I see a 
sccp load...???, also try to register the phone to the pub server to discard 
replication issues,

I would also try with a different sip phone load,

Please let us know your findings,
