I have a system for this create the VLAN structure interface vlan 400 interface vlan 401 ip helper address 10.10.210.10 interface range fast 0/2 - 4 (assuming 3 phones) switchport mode access switchport access native vlan 400 switchport voice vlan 401
ensure the VLAN is shown as up up show ip int brief ensure you have your IP helper address show run interface vlan 401 ensure you can ping the CUCM from the VLAN ping 10.10.210.10 source vlan 401 (or source 10.10.201.1 or whatever your Voice VLAN ip is) usually if I can do these things I have no trouble getting my phones to get IP's from CUCM and register.
