Praveen, In your example you have the user name attribute just set to the first name, the user name attribute must be an exact match for how your source control reports the user for the modified by information. For instance, in my company we use TFS and tfs reports it as domainname \firstinitial.lastname so that is exactly how we have to set up the user name field as well.
