The above error is from the last lines of the log file. It seems that refinement runs fine (I see refinement cycle statistics etc.), however, no pdb or mtz file is written out.

I use CCP4 version 6.0.2.

The very first thing to check is whether or not you have permission to write in the directory where the PDB and MTZ files are to be written to. This might explain an intermittent error of the type you describe.
