We had a group at the university that had a PDP-11 with a fancy graphics frame buffer, they were doing planetary imaging for NASA.  One day the frame buffer went down, and one of the boards had a big HOLE in it.  I gather that a Tantalum capacitor had shorted across the main 5 V power rails.
