First of all, thanks for all the help in the past - I initially thought that
my problem had been solved by replacing the SCSI cable, but it now appears
that it hasn't. Should've done more burns.... ;-)

Devices 1, 3, & 4 share the 8-bit bus. The IBM drive is alone on the U2W
bus. All hard-disks appear to work normally.
I am using the '-dummy' option to try and rule out poor-quality media. In
order to try and stabilize my system as much as possible, I have also:
- Removed XCDRoast from the picture.
- Rebuilt CDRecord 1.9 from its sources, against my current Linux kernel
(2.2.18).
- Replaced the SCSI cable.
- Loosened the drive from it's mounting, and re-aligned it.
- Removed the termination block from the last head on the cable, and
instead activated termination on the CDRom (last physical device).
- Rebooted Linux to non-X init mode, ruling out anything to do with X &
Gnome (which tries to automount removable media).
- Tried 4x, 2x, and 1x, instead of the 8x which my Yamaha 8424S is
capable of.
The command line I now consistently execute is:
cdrecord -v -v -v dev=0,1,0 speed=8 -nofix -dummy -data
fisher-i386-disc1.iso
I have tried altering the speed to 4x, 2x, and 1x, with no discernable
effect.
When I run this command, the burn will work perhaps 25% of the time. The
other times, I get output similar to that shown below. The only real
variation appears to be when the failure occurs - sometimes within the first
1 MB, sometimes after 5-10 MB have been "written" (in -dummy mode), etc...
$ cdrecord -v -v -v dev=0,1,0 speed=8 -nofix -dummy -data
