Well, in 1995 I started to write a ISO-9660 formatter because
I liked to get multi-session support. In order to easier understand
ISO-9660, I did have a look at mkisofs and in turn found many bugs.
After I reported them, I have been informed that mkisofs was
just developing multi-session support and I stopped my own efforts.
Unfortunately, Eric did stop too soon after multi session was halfway ready.
During 1997-1999, I already did the main development work together with
James Pearson. In 1999, I officially got the ownership and in turm did
spend 6 months only on fixing long outstanding bugs.
During the past 5 years, bug fixing for things that habe been in since the
beginning still took more than 50% of the total time.
Mkisofs does not have a clean structure and it has not been well tested
when new code has been added. A major rewrite would be neccesary but this
could lose secret and important small things in the code.

Padding a shrunk file is a real bad idea.
