I run this command, e.g.  "ceph-volume lvm zap --destroy /dev/sdm" when
zapping.

but I haven't run into any locks since going to nautilus, it seems to know
when a disk is dead.  Removing an OSD requires you to stop the process, so I
can imagine anything was running to that.  Perhaps a bad controller, if its
LSI and you are using raid 0 drives like a jbod setup, you could blow up the
virtual disk.
