Hi again all,
I've had to recreate this RBD in a different pool. It seems that the
RBD's in this pool are not functioning on this one machine. If I can
create an RBD and map it, I then get I/O errors trying to mkfs with
either ext4 or xfs. Both of the RBD's in question that are in this pool
can be mapped and used from a different machine (which as far as I'm
aware has been configured in exactly the same way - just has less RBD's
mapped). How can I start debugging this?
The first thing that happened in this pool actually was that the RBD and
associated NFS export was moved from the working machine to the one in
question. All the other RBD's had been working fine and still are on
this problematic box. This particular RBD had a catastrophic filesystem
failure, and I ended up renaming it, and re-creating a new one, first
with the same name, and then afterwards with a different name. The pool
just seems to be broken on this box?
Please let me know what information would be required to look at this
further.

No, no snapshots in this case. It's a brand new RBD that I've created.
Is this format 1 or format 2 image? Can you paste the contents of
/proc/devices and /proc/partitions somewhere?
Also, can you unmap a few of those 15 images that are already mapped
and try to map this one?
