Ignore this part : I wasn't paying enough attention to the osd tree output and mixed osd/host levels. Others have pointed out that you have size = 3 for some pools. In this case you might have lost an OSD before a previous recovering process finished which would explain your current state (in this case my earlier advice still applies).
