The above URL seems very simple though it's good to get its concept across, it seems two ways to tackle it, either using a pdf template with field, then populate it/merge it with data (from db or whatever source), the other, generate a pdf directly, just turn everything in a current html form into pdf format, for what I intend to do for now, I prefer the later approach, any further reading recommendation on this would be appreciated.
