The second two ssn boxes are going to have to be display inline so give them
a class and then display:inline, and set padding on the left and right to
achieve the spacing.
As to the checkboxes, again give them a class of checkbox and adjust the
width to compensate.  You've almost got it.

Okay, I'm still having some issues. My checkboxes are waaaaay far from
my labels.
And, I'm having a hard time figureing out how to then group inputs
that I want to be on the same line (SSN example).
