Then let's move on to questions #2: if Ms. Schiavo had left a
statement saying, "keep me alive no matter what"  who pays?

I bet if you combine all of the people who don't declare, with all of
the people that say there are no measures heroic enough, that's going
to add up to a lot of dough.

Are you willing to give up, say, 10% of your income for this?  20%? 
If not, then who dies, what's heroic, and what's a life worth living?
