We use MSN (there's a new version, 7, you know) and like it. Any problems I've seen could always be traced to connection issues (P2P apps getting busy, Steam deciding RIGHT NOW is the PERFECT time to update HL2, etc). The really nice thing about MSN is the ease of the feature set - my Mom "gets it" and the same software lets us do desktop remote control (which is great for long distance tech support). It's also got file transfer, app-sharing and cam sharing (where we just "publish" our cam image for consumption - for example at birthday parties and such). When you're seeing drops and lags you might want to check your connection, running processes and such. At least in my (admittedly limited) experience all the VC apps work pretty much the same: if one program is lagging they all will (to a greater or lesser extent of course).
