The normal cause of this is the address being checked from multiple
locations.  A POP3 email box can only be accesed by one user at once and
Outlook pops this box up when the account is locked by another user.

We experience this problem here with an account that is checked by a CF
application and an Outlook user.

I did discover a while back a co-workers account that had been hacked was
doing this, some one else was checking his email account from elsewhere
without his knowledge.  If this is the case, try having the password on the
account changed and your problem will vanish.
