I've laughed when called a bitch by my sister, my (female) best friend and even my boyfriend if the shoe really did fit. A stranger on the street who calls me a bitch is asking for the cutting edge of my tongue. It's a word I'd be pretty careful with. Not as offensive as some others, but it's up there.
