It could be a defective print head. When you say the print head is clean, does that mean when you run the print head test, it looks OK?
I ran into very similar problems with XP and a Canon printer. I found that by just restarting the PC, the problem would go away. I think I had a rogue application that would screw up the print driver until a reboot corrected it.
