Well.. yeah.. they are probably referring to their nuts if clutching them and screaming 'oh, bollocks.'... but normally if you hear someone say 'oh bollocks', or 'what total bollocks' or 'that's bollocks' it's kinda like saying 'no way', 'what a pile of shite', 'I don't believe it' that sort of thing. :^)
