Yes but the people who don't have insurance are overhead, see? Which is 
underwritten, at least at public hospitals, by the county and by various 
federal subsidies. The part that is not leads to increased prices for all 
patients. 

By the way, I just checked and if you just google self-pay you get a bunch of 
stuff about the british system. Try "rising medical costs US selfb-pay" or 
something.
