The military leaders who disagreed were removed, and other were put into place.

The military is very clear about who in our society runs the military,
and the answer is civilians. When the civilian leadership made it
clear they did not want to hear about potential problems (by relieving
those who disagreed from command), those left in leadership positions
decided to "shut up and soldier on". Not because they were afraid, but
because that is there job.

BTW, there still are contingency plans that could turn the situation
in Iraq around. They won't be implemented because the solutions are
MILITARY solutions, they will cost a lot in lives and money, require
considerable build up of forces (and focus of the entire country) and
the American people are losing their taste and belief.
