I'm guessing that the cause if ignition could more likely be an ember dropping 
from the cigarette.  But it is unknown from the article.

This speaks to me more of the addictiveness of smoking.  One has been told that 
they shouldn't smoke, but maybe not graphically why.  One thinks to oneself "I 
really need a hit, how bad can it really be...", bums a stick from some 
orderly, goes out to the stair well, enjoys a few puffs and foompft ... flaming 
patient.

True.  But...but...how can you light up before asking?  In my 
experience, things that re flammable usually have a distinct odor.

I dunno, I guess I see your point.  If it were me, I would have at least 
asked if I could smoke.  Maybe I'm being too harsh.
