I'll be honest with you, it's a losing battle. I use a little mail server called XMail and link it in with Spamassassin, I even use sa-learn to try to get SA to learn the patterns in spam, but spammers are so good at it now that you need serious money to do the kind of job that Google does with GMail. Of course, it doesn't hurt to have huge voumes of mail and have users specifically report spam either.
I think I have managed to block at lot of porn, but what I mainly get now is "stock tip" scams trying to pump and dump penny stocks, and those things seem to be impossible to block automatically.
