Many states have guaranteed health insurance plans through Blue Cross. It's best if you can get on them when coming off COBRA which is what I did....but they are certainly not low cost! Any guaranteed health plan (which is often all that us self-employed folks can get) tends to be fairly high with large deductables and co-pays.
