In the end it is a dog eat dog world! Stick out for number one.  We have a
small team but you can bet your paycheck that we all open tickets on
anything we work on. Not like we're highlighting it to our manager but in
the end it's your responsibility for the edits you made, the fact that the
other coder messed things up is mute, you just edited their application.
While documenting the script might be a bit over the top, making note that
you were just in there fixing something is not.

Just believe in yourself, it's not your problem to hold peoples hands
through their mishaps. The client is always right and there may be a time
where they try to cash in on the initial mistake. Documenting it now means
the person answering the phone has some information to work with. I take the
looking out for one another as providing each other with the means to better
assist your clients. In this case, it's the note stating this was fixed on X
date.
