It really depends on your set up - but you'll need a receiver and speakers.

If you've got a better set up than me you can run the HDMI through the
receiver then from that into the TV (better receivers are truly single
points of input).  If the receiver doesn't switch most often you'll run the
HDMI into the TV for video and then use the digital optical cable to run
audio to the receiver.  In the end you just turn the TV speaker right down
and use the receiver's speakers.

I'm hoping (if the car and computer and house hold together) that next
year's tax return will fund a new TV and surround sound system.  Right now
I've no HDMI and really old 5.1 surround (actually 4.0 surround since the
woofer died during the move and one of my rear speakers just blew).
