Muslim countries would be bound to ask him to return according to their own religious laws. This is something that would not be asked of a non-Muslim. It might not ever come up but it is an issue to examine and may well be a reason for his 'alteration' of his past associations with Islam. No country outside of Iran would even think of applying the death penalty to him for 'not returning'.
Muslim fundamentalists, on the other hand, may want him dead more than an average president. This is not a 'let the terrorists win' issue because it's foreign relations between countries, not between groups of individuals. Screw Al Queda/Hamas/Hizballa. They don't count in this. Let me put this in perspective. I'm not allowed into Saudi Arabia because I'm a Jew (no joke, no lie). If I was president, the issue would still be there and while I MAY be allowed in, it would cause friction.
