The slot could definitely be bad... I had one old PC that would only boot
into a blue screen UNLESS I lifted the mobo slightly - wedging a
bog-standard lego brick in there "fixed" it permanently.  This kind of thing
is often caused by hairline cuts in the traces: they work until they heat up
(which only takes seconds really) and then the heat expansion causes it to
lose connectivity.

It could be bad in any number of other ways (too loose - which also would
lose connection when it heats, bad traces, not getting enough power, etc).

You might try upgrading the BIOS if that's an option.  Sounds like you did
already but also make sure that you've disabled the onboard video (you can
reenable later if you need to), set your AGP aperture size low or off to
start (makes sure you're not actually hitting a weird RAM issue), make sure
that AGP isn't overclocked or adjusted in any way (in fact it may be best to
try setting your system to "safe" bios defaults if there's an option).

Silly, but if you've had the system a while without using the slot you may
want to give it a good cleaning.  Accidentally trapping dust or a hair in
the slot can short-out the traces.
