I think it's the second part.

But it's an interesting question if taken seriously.  For example:

"Registration for Selective Service is also required for various
federal programs and benefits, including student loans, job training,
federal employment, and naturalization."

"The Selective Service System provides the names of all registrants to
JAMRS for inclusion in the JAMRS Consolidated Recruitment Database.
The names are distributed to the Services for recruiting purposes on a
quarterly basis."

If you're not healthy you can't fight so it could be argued that
healthcare registration is an extension of the selective service with
the issue, of course, being that healthcare costs money but then
that's also called a tax.

So one solution might be that someone could refuse health care but
that would have significant penalties a la the selective service
