First things first, a saying I learned:
"If nobody died, anything can be fixed." - This came from my boss
before he left to buy his own company.  I seriously questioned what
kinds of sedatives he was on but he wasn't, he just looked at the
business with this view, dealt with the issues in a controlled but
expedient manner.

I've had programs start great and then fall apart.  I've made upgrades
that have dropped entire customer catalogs because the table
structures didn't match.  Hell, I've had a job where one project I
aced and the other cost me the job.

on.  Nothing else I can do.

Testing isn't an easy thing - I used to HATE our tester until I
realized that he was doing his job WELL.  I can see where you think
it's all on you but it truly isn't.  You said the QA team decided not
to do any regression testing... bad idea (any time) but it doesn't
sound like you made it.

And the truth is, LOTS of companies are making mistakes like that.
The company that provides our main payroll/hr/benefits/accounting
system (which is what we DO for our clients, btw) had to rush out a
release to accommodate the Social Security split.  Since that upgrade
on 1/1 (yes, I had to install it on New Year's Day, it had to be in
before the first payrolls of the year processed) they have released 4
more upgrades and I have personally identified 3 issues that are being
worked on for the next!

Mistakes were made and new need to be corrected.  My key advice is
whatever you do, do not rush the fixes and produce more problems!
Remember, "as long as nobody is dead, anything can be fixed!"
